COVID-19 Testing

IMPORTANT INSTRUCTIONS:

  • Upon arrival for your test, park in an empty space on the east side of building adjacent to the drive thru.

  • Please do not leave your vehicle and wear a mask at all times.

  • Call the pharmacy at 254-629-1791 to notify us of your arrival.

  • Please note that it is the patient's responsibility to choose the COVID-19 test that is appropriate for their situation. Thank you for your understanding.

Submitting Insurance Claims:

  • The Pharmacy Does Not Bill Insurance for Your Test.

  • Find below the forms needed to submit to your insurance.

  • Check with your insurance prior to booking to ensure reimbursement.
